Lateral malleolus

The lateral malleolus, or inferior aspect of the fibula is generally very straightforward to identify. In some individuals however, this feature may be obscured by the presence of oedema for example. Begin by palpating the anterior border of the lateral malleolus. This border provides the insertion for the anterior tibiofibular ligament superiorly and the insertion point for the anterior talofibular and calcaneofibular ligaments distally. The exact attachment point of the calcaneofibular ligament may be palpated as a small groove on the anterior border of the bone as you move toward the apex of the lateral malleolus.

To palpate the posterior border of the lateral malleolus, continue to run your finger posteriorly from the apex. To correctly identify the posterior border, ensure that the peroneal tendons are fully relaxed to prevent them from sliding forwards and obscuring this anatomical feature. Note the posterior border provides attachment points for the posterior tibiofibular and posterior talofibular ligaments.
